How are habits formed and broken in terms of behaviorist learning theory?

I am still compiling for a future research paper on Sexual Behaviors of sexually abused children, with an issue of excessive masturbation.  Any suggestions on the following two questions: I'm really needing only 250 words per question.

Question 1      Habits can be formed or broken by the conditions of behaviorism.  Apply at least 3 terms from one of the early behaviorism theories to a habit that you would like to form or to break.  With the 3 terms, plan a strategy to form or to break the habit.

Question 2      Use classical conditioning to produce consistently some startle response (reflex) in an unsuspecting friend at an abnormal signal (produces the reflex).  Start with your friend's reflex response and a signal will produce that reflex response; then choose another signal for the reflex.  Clearly label the UCS, UCR, CS, and CR.
